CalAIM / Re-entry
The Nevada County Sheriff's Office offers Re-entry services to all incarcerated persons housed at Wayne Brown Correctional Facility and Truckee Substation. Nevada County Sheriff’s Office partners with other County agencies such as Department of Social Services, Behavioral Health, Information Services, and Probation in addition to Community Based Organizations, to provide whole-person care. In connecting with these agencies, it creates a linkage not limited to resources such as housing needs, medical necessities, supervision and employment in hopes to reduce recidivism.
Re-Entry Services:
The Nevada County Sheriff’s Office has two Correctional Re-entry Coordinators (CRC) /Social Workers who offer services to all incarcerated people housed in our facilities. The CRC will meet with the incarcerated individual, explain the benefits of Medi-Cal and associated programs. If an incarcerated person agrees to apply for Medi-Cal, the CRC will coordinate a meeting with a Department of Social Services (DSS) eligibility worker. The eligibility worker will meet virtually with the incarcerated person via dedicated “CalAIM tablets”. The DSS eligibility worker will go more in depth about State or County sponsored programs such as CalFresh, General Assistance, CalWORKs, or the Emergency Relief program.
Shortly after intake into the Facility, the CRC will schedule a meeting with the IP. This meeting will consist of a self-evaluation that creates a discharge plan. The self-evaluation begins with a series of questions related to functional needs, basic needs, education and employment, housing, benefits through sponsored programs, and health and wellness needs. Once these needs are identified, the CRC will begin coordinating with Jail Medical Staff, Behavioral Health, Enhanced Care Managers, and Community Based Organizations. The goal is to stabilize any mental and physical health needs, connect the individual with community-based organizations to set the individual on a path to success.
Transitioning Back to the Community:
The goal of continuous coordination is to ensure that the Incarcerated Person is connected with the resources that will assist them in the community prior to their release. The CRC coordinate “warm handoffs” with the community-based programs and other agencies by scheduling a meeting for an introduction to services. It is important that the Incarcerated Person is aware of the services that will continue to assist them in the community.

A CRC will meet with the Incarcerated Person within their first 3-5 days.
- How can an incarcerated person request a referral?

Incarcerated Persons can request referrals through the kiosk, a Correctional Officer, any jail medical staff, or directly to the CRC.

Mental health services, Enhanced Care Management, Inpatient and outpatient treatment facilities, Medical assisted treatment programs, housing support, food distribution programs, insurance benefits, employment services, education programs, veteran’s services, vital records and identification, and more
